Position Summary The Perinatal Family Advocate provides supportive case management services to perinatal families in the Bayview Hunters Point community, helping them access and utilize essential resources. This on-site role at the Bayview YMCA includes administrative coordination, family engagement, and outreach to strengthen connections with community organizations, city departments, and HOPESF sites such as Alice Griffith, Sunnydale, and Huntersview. A key focus of this position is to promote family safety, stability, and well-being by proactively engaging families and connecting them to on-site services and community resources. Successful candidates will have strong outreach and relationship-building skills to foster inclusion and cohesion among families and service providers. This role follows a Monday–Friday, 9:00 AM – 5:30 PM schedule, with some evening and weekend hours as needed. Travel is required. Job Responsibilities Case Management & Client Support Conduct intake assessments, develop service plans, provide referrals, and complete post-service evaluations to document measurable outcomes. Maintain an active caseload of 25-30 clients, ensuring ongoing support and engagement. Connect families with relevant information, resources, and support services. Conduct regular outreach to recruit and enroll participants in program services. Assist clients in navigating available programs, organizations, and city services. Program Coordination & Community Engagement Build and maintain professional relationships with collaborative partners, community organizations, and service providers. Participate in and facilitate workshops, support groups, and community events as needed. Support and implement family engagement activities, including early literacy initiatives. Liaise with external agencies to coordinate services and ensure comprehensive support for families. Administrative & Compliance Responsibilities Maintain accurate and organized client records, ensuring timely documentation, data entry, and reporting. Compile data and reports as required by funding agencies and organizational guidelines. Adhere to confidentiality policies and procedures, ensuring client data and personal information are protected. Teamwork & Professional Development Participate in weekly, monthly, and bi-monthly meetings, sharing updates on client needs and program objectives. Provide support to team members and collaborate on program initiatives. Demonstrate conflict resolution and de-escalation skills to ensure a safe and supportive environment. Qualifications Minimum 18 years old. High School Diploma or equivalent. Strong interpersonal, written, and verbal communication skills. Proficiency in MS Office 365 or similar software. Availability to work Monday–Friday, with some nights and weekends as needed. Experience in case management, client follow-up, and program reporting. Access to reliable transportation. Preferred Qualifications Experience in client advocacy, trauma-informed practices, and domestic violence counseling. Bilingual proficiency in Spanish, Cantonese, Mandarin, or other languages. Work Environment & Physical Demands This position operates in a community-based setting, requiring both office work and outreach in the field. It involves frequent interaction with clients, partner organizations, and community members. The role may require standing, walking, and sitting for extended periods, as well as occasional lifting of materials up to 25 pounds. Travel within the community is necessary, and some evening and weekend work may be required.
