Senior Community Health Nurse - 64028368

This is an open competitive career service Senior Community Health Nurse position working in the Lee County Health Department. This position is responsible for planning, monitoring, evaluating, and providing services on a countywide basis in outreach immunization clinics, employee health, and the health department. This position is under the direct supervision of the Nursing Program Specialist.

This Senior Community Health Nurse is responsible for:

  • Planning, monitoring, evaluating, and providing services on a countywide basis in outreach immunization clinics, employee health, and the health department.
  • Assisting with immunization program activities/services for the agency according to the current edition of the Epidemiology and Prevention of Vaccine-Preventable Diseases (CDC's The Pink Book), the ACIP Recommendations, and guidelines from the State of Florida Immunization Program.
  • Promoting and maintaining the health of individuals, families, and the community through appropriate measures.
  • Providing comprehensive nursing; vaccine administration in all FDOH-Lee clinics and community settings.
  • Adhering to immunization program guidelines; correct use of VFC (PC01), Uninsured/Underinsured (PC17), purchased vaccines (PC05), and Hepatitis (PC09) vaccines.
  • Ensuring proper documentation of immunization services into Florida SHOTS and HMS.
  • Maintaining proper storage and handling of all vaccines, as per recommendations of the current edition of the CDC Vaccine Storage and Handling Guide.
  • Utilizing monthly reports (Reminder Recall/Televox) to send reminders to clients who are coming due/past due for vaccinations.
  • Administering vaccines for overseas travelers and maintaining up-to-date overseas travel information and being knowledgeable of the appropriate vaccines to administer relevant to the area of travel.
  • Providing statistical reporting as required and/or requested.
  • Communicating anticipated vaccine and supply needs with Immunizations supervisor, monthly and/or as needed.
  • Serving as a subject matter expert and resource person for Immunizations and Employee Health, providing technical assistance and educational support to nursing staff at DOH-Lee, and as needed to private providers and area CHDs.
  • Maintaining complete and current knowledge of all aspects of the immunization program including, but not limited to; childhood (Vaccines for Children-VFC), adult, travel, and providing outreach services.
  • Conducting workshops and sending educational CEU opportunities to staff.
  • Performing immunizations skills checkoffs for clinical staff.
  • Promoting access to vaccines in the community for adults, children, and special needs/high risk populations through local coalitions and other community partners.
  • Reviewing DOH-Lee Employee Health protocol, as necessary.
  • Attending pertinent conferences, meetings, and events as requested or permitted.
  • Responding to public inquiries regarding immunization guidelines and recommendations in collaboration with DOH-Lee's Public Information Officer (PIO).
  • Responding to DOH employee inquiries regarding Employee Health policies and recommendations.
  • Planning, coordinating, and monitoring outreach programs to ensure efficiency and benefit to the community.
  • Assisting with the outreach immunization program for children and adults as needed.
  • Aligning outreach initiatives with the county health department, health care providers, health management organizations, senior centers, active coalitions, nursing homes, and assisted care facilities.
  • Conducting educational presentations and implementing immunizations campaigns to increase community awareness.
  • Completing and submitting monthly outreach documentation.
  • In conjunction with the Public Information Officer, maintaining and distributing current list of outreach clinic events and sites to appropriate persons and agencies, conducting educational presentations to enhance awareness while reinforcing the importance of vaccination.
  • Assisting with health fairs, community organizations, and specific public health events.
  • Performing quality improvement activities to monitor the effectiveness of outreach activities.
  • Performing Employee Health nursing duties as outlined in FDOH Lee IOP 13-3.
  • Conducting Employee Health clinic for all new hires and completing necessary paperwork.
  • Reviewing and maintaining employee health records.
  • Educating employees on mandatory and recommended immunizations.
  • Administering employee immunizations and/or facilitating acquisition of necessary titers.
  • Assisting with coordination of needle stick exposures and post exposure prophylaxis.
  • Advising employees about job-related health risks and exposures.
  • Coordinating employee flu clinics at all FDOH-Lee sites annually and other vaccines, as necessary.
  • Assisting the Public Health Emergency Preparedness Program with fit testing with N-95 masks.
  • Conducting TB risk assessments annually or as needed.
  • Updating employee spreadsheet (notes whether staff is new, current, or separated) and immunizations provided, monthly.
  • Monitoring climate-controlled storage (Verilert) for vaccines by responding to alerts and alarms in a timely manner and ensuring proper temperature logs.
  • Responding to special directions and instructions during emergency operations, secondary person responsible for the Vaccine Storage Emergency Response Plan.
  • Timely and accurate documentation of leave and attendance records; biweekly timesheet and daily employee activity records (EARS).
  • Assisting with student and volunteer orientation by precepting and evaluating students through observing their performance in various settings, monitoring their work, and providing input to students' clinical coordinator.
  • Performing other duties as assigned.

  • Ability to frequently bend, kneel, reach, and sit or stand for long periods of time.
  • Ability to lift and carry up to 50 pounds as needed.

Licensure/registration/certification requirements:

  • Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.

Other job-related requirements for this position:

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.
  • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.
  • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
  • This position will abide by all state and federal laws, rules, and DOH policies and procedures.
  • This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.
  • Must have current CPR with AED (BLS) certification or obtain such certification during employment.
  • Must be willing to undergo FIT Testing.
  • Successful completion of a drug test is a condition of employment.
  • This position is classified as sensitive. This individual will perform tasks defined as sensitive and handles confidential information. Background screening is a condition of employment.
