Schedule: Full-Time/Day Shift. 8am-5pm (weekend coverage rare)
Location: AdventHealth Hospice
The role you’ll contribute:
The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for hospice consultation and or admission to AdventHealth Hospice Care. The Admissions Nurse assesses the appropriateness/qualifications of the patient for Hospice services and informs the patients, caregivers, and family members of the hospice services available with the organization. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care patients who have been admitted to an AdventHealth hospital.
The value you’ll bring to the team
· Provides information and available resources to patients/family/caregiver for admittance onto
Hospice services.
· Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
· Travels to the patient’s location or hospital room of patient who has been referred to Hospice.
· Partners with the hospital Case Management staff member to meet the needs of the patients and hospital staff to gain new referrals.
· Assesses the patient and family’s emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified.
· Coordinates efforts for patient treatment and promote quality improvement and educational efforts.
QualificationsThe expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
·Diploma or Associate Degree from an accredited school/college of nursing required.
·Minimum of 1-year Registered Nurse experience in Hospice and/or Acute Care Nursing experience.
EDUCATION AND EXPERIENCE PREFERRED:
·Bachelor of Science in Nursing
·Hospice experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
·Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
·Maintain a current, valid Illinois Driver's License in good standing with a safe driving record and a reliable vehicle.
·Maintain valid personal automobile insurance.
