And we do it all with heart, each and every day.Role DescriptionThe House Calls Medical Assistant/Scribe (CIS/MA) fulfills a crucial role for our in-home care teams; supporting logistics, clinical care, and documentation in our innovative home-based care program. The CIS/MA works directly with a House Calls Provider on a daily basis to help prepare patient visits, document visits in the EMR accurately, identify and bridge gaps in care, and advocate for patients and families when patients return from the hospital or experience other challenges.ResponsibilitiesPrepare daily visit schedules, including routing and logistics planningAccompany House Calls Provider on home visitsIn accordance with state regulations, check vital signs, administer vaccinations, perform point of care tests, blood draws for lab testingAssist in reconciling patient medicationsSupport patient and family educationDocument Patient Encounters in a structured note Include the history of the present illness, assessment, plan, and physical examAssign appropriate CPT and ICD-10 codesPrepare After Visit SummariesConsult with provider to ensure accurate and specific documentationImprove Clinical Documentation, as appropriateRequest and review medical recordsLeverage Oak Street's population health tools to support clinical documentation improvement in a value-based care modelPrepare for and support Daily Huddles and Clinical Documentation ReviewsConsult with provider on clinical documentation opportunitiesAdministrative support for provider and care teamProcess and manage orders and referrals, including follow-up callsAddress tasksMaintain supplies and equipment, including routine quality control checking (i.e. infection control measures and logs, recruitment and checks for expired medication and supplies)Participate in daily huddles and weekly interdisciplinary team meetings to discuss patient care and program operationsOther duties as assignedWhat we're looking forGenuine passion for primary care and reaching vulnerable patientsComfort navigating visits in patient homesDemonstrated ability to collaborate effectively in a team settingWillingness to learn and be accountable for visit documentation and workflowsExcellent communication, follow-up, teamwork, and problem-solving skillsDesire to be a part of a fast-paced, innovative, quality-driven organizationRequired QualificationsState or national Medical Assistant certification (as required by state), or graduation from an accredited medical assistant courseCPR or BLS CertificationMinimum 1 year experience as a Medical AssistantMust pass all clinical competencies before supporting patient care, phlebotomy experience a plusDaily access to a vehicle with valid license and insuranceFlexibility to travel throughout service areaElectronic Medical Record experienceComputer skills: Ability to quickly navigate and use multiple computer programs to include, but not limited to: Gmail, MS Word or Google Docs, Excel or Google Sheets, etc.Bilingual (English and Spanish) a plusUS work authorizationWhat does being "Oaky" look like?Radiating positive energyAssuming good intentionsCreating an unmatched patient experienceDriving clinical excellenceTaking ownership and delivering resultsBeing relentlessly determinedAnticipated Weekly Hours40Time TypeFull timePay RangeThe typical pay range for this role is:$18.50 - $38.82This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
